依據本校4月7日防疫工作會議決議，4/2~4/6日期間曾前往政府公告之11個熱門風景區/景點者(阿里山森林遊樂區、花蓮東大門夜市、嘉義文化路、台南關子嶺、虎頭埤、烏山頭水庫及湖境度假會館等埤塘風景區、高雄興達港、旗山老街、雲林北港朝天宮、屏東縣南州鄉以南（含墾丁觀光景點）)，請務必填報線上問卷https://forms.gle/gnKrqPASpV3c5ogz8，並自主健康管理14天，自主健康管理期間，請依下列規定辦理：
【1】職員工上班應全程配戴口罩，如欲請假者，請依人事室4/6日公告事項辦理。
【2】學生上課應全程配戴口罩，如欲請假者，課業部分請依教務處規定辦理，請假事宜請洽生輔組。(請假事由：4/2-4/6旅遊史)
【3】教師建議採遠端遠端視訊上課，如採實體課程，教師應全程配戴口罩。

【5】自主管理期間每天量測體溫2次，外出時務必配戴口罩，如有身體不適請撥打免付費防疫專線1922，依指示立即就醫，請勿到校上班上課。
如經醫師確認為一般上呼吸道感染，且無服用退燒藥物亦沒有發燒症狀24小時後，仍可回校上班上課！

If you have visited these 11 locations warned by The Central Epidemic Command Center (CECC) during the spring break (Kenting, Alishan Forest Recreation Area, Hualien Dongdamen Night Market, Chiayi Cultural Road, Tainan Guanziling, Hutoupi, Wushantou Reservoir Scenic Area, Kaohsiung Xing Da Harbor, Qishan Old Street, Yunlin Beigang Chaotian Temple), please fill out this questionnaire (https://forms.gle/gnKrqPASpV3c5ogz8) and conduct 14 days of self- health management. Wear a mask during self-health management. If you feel unwell, to call the toll-free Communicable Disease Reporting and Consultation Hotline, 1922 (or 0800-001922), and inform the physician of any history of travel.

- Please wear a mask during the class.
- Maintain social distancing( 1.5 meters indoor; 1 meter outdoor) and washing your hands correctly.
- Students requesting leave to contact the Student Services Division.
